The Patient Who Didn't Improve Until We Fixed His Breakfast

And this wasn't even my patient.

He was a close friend from school. We now live at two ends of India, but we still have the luxury of meeting twice a year because of a common interest.

The last time we met, something caught my attention.

There was a dark patch over his neck and over both sides of his face. This was classic of a condition known as acanthosis nigricans. It strongly suggested metabolic disease and the cause being insulin resistance.

At first I didn't say anything, I was wondering how he would take it. Finally, I gathered the courage and asked him about his food habits.

He had just crossed 50 and lived alone after losing both his parents a few years ago. He works from home, doesn't keep regular hours, and like many people living by themselves, food had slowly become whatever was easiest. I discovered his meals consisted largely of bread. Not only breakfast but the other meals would invariably have bread.

Ironically, he hardly ate outside food. Years ago, he had a bout of severe hepatitis while he was in college, and that experience made him very careful about eating out.

I suggested he get his fasting insulin test and HOMA-IR. Both of which are tests which are rarely prescribed but is very useful in this case.

I also recommended he do just one thing.

"Stop the bread completely." That was it. I wasn’t even going to prescribe any medicine or a complicated diet. No long list of foods to avoid. Just one simple change.

The next time we met, I could see the difference.

Slowly over the next few months, the pigmentation started fading. He told me he had more energy, was sleeping better, and simply felt better than he had in a long time.

This reminded me of something I see over and over again.

Often we seek for a miracle cure while ignoring the everyday habits that quietly damage our health.

Sometimes the first step towards healing isn't popping a pill. It could just be what's on your plate that’s working against you.
